DEVELOPING AN INDIVIDUAL HEALTHCARE PLAN FOR YOUR CHILD

 To ensure we can keep your child safe and ready to learn at school we need to know if there are any health issues that need support or advice if there is a medical emergency. Medical issues may include ADHD medication, food allergies, epilepsy, physical difficulties etc.

If this is the case please complete the plan and hand it into the office.

If you have any doctors letters that may be useful to help with the plan and need to shared with the staff in their team please send them in.

Find attached a template of the care plan. This can be downloaded or you can get a copy from the office.
